Health Information Exchanges

In an effort to provide you the safest and best care possible Foundation Health Partners (FHP) participates in several Health Information Exchanges (HIE). FHP’s participation in these HIEs allows health care professionals to appropriately access and securely share medical information electronically.

Sharing this information enables your FHP doctor or health care provider to confidentially provide vital medical history, no matter where you’re receiving care, with another participating health care provider. It offers safer, more effective care that can be tailored to any unique medical needs you might have. Participation in these HIEs is voluntary and you can easily opt out, any time.

Benefits

Participation allows authorized health care providers who share in FHP’s HIEs, or is a member of a health information exchange that is connected to these FHP HIEs, to electronically access and share health information as set forth below, unless you opt-out:  

  • Health information that is shared through these HIEs is the most current information available and may include information related to treatment you received from any provider who is connected, either directly or indirectly, to the FHP HIEs, including out-of-state providers.  
  • The health information that will be shared through these HIEs includes information about your diagnoses, test results (like x-rays or laboratory), and medications that have been prescribed to you. 
  • Every effort is made to restrict sensitive information such as HIV test results, mental/behavioral health records, controlled substance treatment programs, and genetic/hereditary test results.   

Opt-Out

If you would like to opt-out of any of the HIE that FHP participates in, please request, complete, sign and return the FHP Individual Health Information Exchange Opt Out form. If you opt-out, of any HIE, but then wish to opt-back-in, please request, complete, sign and return the FHP Individual Health Information Exchange Opt In form.

Additional information

Fairbanks Memorial Hospital (FMH), Tanana Valley Clinic (TVC), and Porter Heart and Vascular Center (PHVC) all have separate electronic health records. This means they connect to HIEs separately, and do not always participate in exactly the same HIEs. It also means you can only opt-out of the HIEs connected to the clinical organizations you have been a patient at. Here is some information on the HIE for the different clinical organizations: 

  • FMH, TVC, and PHVC all participate in the Alaska State, HealtheConnect Alaska HIE.
  • Most larger clinical organizations in Alaska participate in the HealtheConnect Alaska HIE. 
  • It is a statewide HIE that allows doctors and nurses at any participating facility to search the HIE for clinically relevant information from all other participating organizations where you were a patient and vice versa.    
  • FMH participates in the following exchanges:
    • The Sure Scripts Prescription Exchange. This HIE is for the exchange of prescription medication information.
    • Allows FMH doctors and nurses to search the HIE for your prescription history from other organizations where you were a patient.
    • Allows other clinical organizations where you go for health care to lookup your prescription medicine information from FMH, if they participate in the same HIE.
    • If you were traveling and needed medical care, and the hospital or clinic you visit participates in an HIE connected to the Sure Scripts Exchange, they would be able to access your prescription medication history from FMH and other participating organizations to help choose the right treatment for you.    
  • TVC participates in the following exchanges:
    • The NextGen CareQuality / Sure Scripts Exchange is for the exchange of both clinical information and prescription information.
    • Allows TVC doctors and nurses to search the HIE for clinically relevant information from other organizations where you were a patient.
    • Lets other clinical organizations where you go for healthcare to lookup your clinical and prescription medicine information from TVC, if they participate in the same HIE.
    • if you were traveling and needed medical care, and the hospital or clinic you visit participates in an HIE connected to the CareQuality / Sure Scripts Exchange, they would be able to access important health information from TVC and other participating organizations to help choose the right treatment for you.     
  • PHVC participates in the following exchanges: 
    • The NextGen CareQuality / Sure Scripts Exchange is for the exchange of both clinical information and prescription information.
    • Allows PHVC doctors and nurses to search the HIE  for clinically relevant information from other organizations where you were a patient.
    • Lets other clinical organizations where you go for healthcare to lookup your clinical and prescription medicine information from PHVC, if they participate in the same HIE.
    • If you were traveling and needed medical care, and the hospital or clinic you visit participates in an HIE connected to the CareQuality / Sure Scripts Exchange, they would be able to access important health information from PHVC and other participating organizations to help choose the right treatment for you.
