We are very fortunate and grateful to welcome you to our practice, and we look forward to providing you with a pleasant and positive experience at Sportmedicine Fairbanks. To help your first visit go smoother, please read through all of the following information.
Patient Information and Registration
Please take a few minutes to download and complete the forms on the right sidebar of this page. Completing these forms prior to your visit with us will expedite the registration process. Please bring these completed forms with your to your visit.
At the time of your first visit, you will be asked to present your insurance information (we are happy to photocopy your insurance cards) and we request payment in for for the initial visit, unless financial arrangements are made in advance (see next paragraph). We will file the insurance claim for your medical services with your insurance carrier. If you utilize any of the following carriers, please bring the appropriate paperwork to your visit.
- Veteran's Administration (or) Tricare/Tricare for Life - bring written authorization for the services (authorization should be specific to the date of service).
- Workers' Compensation - bring copy of injury report
- Federal Workers' Compensation - bring the "CA-" form from your employer (green paper)
If you are utilizing active, valid benefits from the following carriers, you will not be asked to pay for your visit at the time of service; however, any copay that your are responsible for will be collected at the time of the visit:
- Workers' Compensation (including federal)
- Veteran's Administration
- Tricare/Tricare for Life
- Premera Blue Cross (we are a preferred provider for Blue Cross of Washington/Alaska)
Your Medical Records
If you have received care elsewhere, please bring a copy of ALL medical records (to include chart notes, xray films, diagnostic study results and any other records available) to your visit. You will need to notify the other health care provider that you need the records, and most facilities require 24-48 hours to prepare your records and will require a written authorization from you to obtain the records. If xrays were done at Fairbanks Memorial Hospital the films need to be picked up from the hospital.

We are committed to you, our patient, and anticipate that you will occasionally have questions or concerns that need to be addressed outside of your scheduled appointment times. We make every effort to return your calls the same day, but if circumstances arise that make it difficult to do so, we will contact you to advise you of the status of your request. For your safety, we are unable to give medical advice over the telephone.
