We are committed to providing our patients with the best possible care and convenience, and this new patient portal is a significant step in that direction.

Benefits of Our New Patient Portal:

• User-Friendly Interface: Designed with user-friendliness in mind. Navigating through health records, appointment scheduling, and communication with healthcare teams will be more intuitive than ever before.
• Messaging: Patients can ask their doctor and care team a question, share an update, and/or send an attachment 
• Appointments: View and manage appointments 
• Medications: Request refills and/or renewals and review prescription medications 
• Health Record: View visit summaries, lab results, and other documentation 

How to request your historical health records:

1. Start by selecting "Health Records" on the Patient Portal Homepage.
2. Once on the Health Records page, click the link labeled "Request Update Records."
3. Choose either a specific date range or opt for the complete historical health record.
4. Click on the "Request" button.

Please be aware that if your request is completed correctly, you will receive a confirmation message stating "Request Received." Once your records request has been processed, the information will be sent to your Patient Portal, and you will receive a notification.

Foundation Health Partners offers secure electronic Patient Portals at Tanana Valley Clinic (TVC), Fairbanks Memorial Hospital (FMH), and Porter Heart Vascular Center (PHVC). This allows you to manage your health care and access your health information, whenever you need it.

*Enrollment in the TVC portal is currently not available for minors aged 12-17 years old. Minors in this age group are automatically unenrolled when they reach 12 years of age.
