HEART of Excellence

The HEART of Excellence program represents a collective commitment to excellence and living out FHP’s HEART service behaviors: 

Honest
Engaged
Accountable
Respect
Team-player

This employee recognition program is designed to celebrate the moments of excellence with our patients, residents, visitors, and each other. All employees receiving nominations are recognized in their department. Each quarter, two employees, one clinical and one non-clinical, are chosen from all the nominations to receive the HEART of Excellence Award.

Daisy Award

The DAISY Award started at Foundation Health Partners in January 2011 as a way to honor RN’s who exceed the following criteria:  leads by example, sensitive caregiver, team player, dedicated mentor, true patient advocate, insightful caregiver.  Patients, family members, providers, employees, and others are encouraged to submit detailed nominations (include a story) for nurses.

The DAISY Committee chooses an Honoree three times a year.
