FHP Patient Portal
Effective June 1, 2026 care team messaging is disabled on the TVC and PHVC patient portals. Effective June 30, 2026, the TVC and PHVC patient portals will be discontinued as we transition to a new FHP system‑wide patient portal.
Your new patient portal does not pull forward your historical medical record. If you want to request your historical medical records please visit:

What is the Patient Portal?
The FHP Patient Portal is a secure online resource to manage your care across the entire FHP health system at your convenience. Key benefits of the portal include:
- View upcoming appointments
- Secure messaging with your care team
- View lab & radiology results
- Manage medications
You must have received services at Fairbanks Memorial Hospital, Tanana Valley Clinic, an FHP Hospital-Based Clinic to enroll in the FHP Patient Portal.
If you do not yet have your account set up or if you need help receiving an account invitation for the FHP patient portal, please email the FHP Patient Portal Team and include your full name and a current phone number so we can assist with the account activation process.
If you already have your account set up but need some help accessing or using it, please call 877-621-8014. Please be ready to provide the customer support representative your full name, date of birth and current email address.

Request to Amend or Supplement Information
If when reviewing your medical records, you see health information you believe is incorrect or incomplete, you may ask us to amend your record.
You may submit a request to amend your medical information by contacting Health Information Management via email at fmhrecords@foundationhealth.org, call us at (907) 458-5450 or in person at our office at TVC, 1001 Noble Street, Suite 400 M-F from 8am-4:30pm.
