Effective June 30, 2026, the TVC and PHVC patient portals will be discontinued as we transition to a new FHP system‑wide patient portal. If you enroll in the FMH Patient Portal today, you’ll be automatically enrolled in the new portal when it is launched, giving you access to all your FHP health information in one convenient place.

Foundation Health Partners offers secure electronic Patient Portals at Tanana Valley Clinic (TVC), Fairbanks Memorial Hospital (FMH), and Porter Heart Vascular Center (PHVC). This allows you to manage your health care and access your health information, whenever you need it.

*Enrollment in the TVC portal is currently not available for minors aged 12-17 years old. Minors in this age group are automatically unenrolled when they reach 12 years of age.

If when reviewing your medical records, you see health information you believe is incorrect or incomplete, you may ask us to amend your record. 

You may submit a request to amend your medical information by contacting Health Information Management via email at fmhrecords@foundationhealth.org, call us at (907) 458-5450 or in person at our office at TVC, 1001 Noble Street, Suite 400 M-F from 8am-4:30pm.
