TVC Health Information (Medical Records)

We process our requests on the basis of which we receive them, but we strive to accommodate our patients to the best of our abilities. If you are a Fairbanks Memorial Hospital (FMH) patient and would like your records, please contact the FMH medical records office. 

How to Request Records

Third Party and Personal Requests

Fill out the medical records authorization form completely, sign and date. 

All requests must come with a copy of an official photo ID (typically a driver’s license).

Submit your request via one of the following:

  • Email (secure)
    Send an email along with a copy of your photo ID and a completed authorization form.
  • US Mail
    Mail your request, a copy of an official photo ID, and completed authorization form to:

  • In-person
     If you plan to come to our service window during business hours please have your official photo ID (typically a driver’s license) ready. An authorization form will be given to you to complete.

Physician Requests (Continuity if Care)

Upon request, records can be sent to other physicians involved in your care. If your physician is out of the Fairbanks area please provide their contact information so we may better assist you. *

Submit your request via one of the following:

  • Email (secure)
    Send an email containing the necessary information for request
    • Patient name and date of birth, facility or provider’s name, their contact information, and what records you would like us to send.
  • Voicemail
    Leave a message on our phone line 907-458-2647 with detailed information about your request. Your physician’s office may call us as well.
    • Patient name and date of birth, facility or provider’s name, their contact information, and what records you would like us to send.

*Certain sensitive information may not be disclosed until a request form is signed by the patient/personal representative

Frequently Asked Questions

Q: What is the turnaround time for records requests?

All requests are prioritized with patient care as top priority status. Requests will be completed based on priority status and receipt date. Generally all requests will be completed within 30 days of receipt.

We are a hybrid facility, meaning we have records in both paper and electronic format. Patients seen prior to 2004, and who are currently being seen, will have a hybrid record. Patients established after 2004, will have an electronic record only. Processing times may differ depending on your personal record history.

Q: How do I find out if my child’s shot records are up to date?

To confirm your child’s shots are up to date over the phone, please contact the Pediatrics Department at 907-459-3520.

Q: How do I get copies of my child’s immunizations?

Parents and legal guardians can sign a records release form in order to obtain copies of immunization records. You can email us (preferred) or call and ask to have an electronic request form sent to your email. Be sure to provide us with your child’s name, date of birth, and a good email address. We offer an electronic signature option, so you can complete this on your phone or computer. You can also print the FHP Release of Information Form from this page, which can be filled out and emailed to us or via fax at 907-459-3566. Please include a copy of your photo ID with the Release Form, when emailing or faxing your request.

Alternatively, parents may request records in person at our medical records window, located on the first floor of the main clinic (Noble street), to the left of the main reception desk.

Q: How do I get copies of the immunization record sent to my child’s school/daycare?

Parents/guardians may request that we fax immunization records to a local school or daycare. This can typically be done without the need for a release form. Call us at (907) 458-2647 or email us with your child’s name, date of birth, and what school/daycare you would like us to send the records to.

Q: Can you send records directly to my new provider?

Yes, we can. Either fill out and return our release form, give us a call at 907-458-2647 or email us with your name, date of birth and where you would like us to send the records to. We will process the request as soon as possible.

Q: I need to request records from my previous provider to be sent to TVC. How do I do this?

You can either contact your previous clinic to send us the records, or you can fill out and return one of our release forms and we will send it, requesting the records on your behalf. 

You can give us a call at 907-458-2647 or complete our release form, and email it to us. Please be sure to include a copy of your AKDL.

Q: Can you email my records to me?

Yes, we can. Just fill out and return our release form with your e-mail and we will process this request as soon as possible. Photo ID is required prior to release of records through email.

Q: Can you fax records to me?

No, we can only fax records to secure fax lines such as another clinic. However, we can email, mail or have your records ready for pick up.

Q: Where is Medical Records located in TVC?

We have a pick-up window on the first floor of the main building, the last window on the left side of the large reception desk. We can take your request, answer questions, and have you pick up your processed records from there.
