How We are Improving Patient Safety
Patient Identification
Accurate patient identification is essential to patient safety. We ensure that every treatment, test, and medication is provided to the correct individual by using two patient identifiers, verifying ID bands, confirming information verbally, and matching orders to the patient’s records.
Hand Hygiene
Hand hygiene is a very important infection prevention measure, as 80% of infections are spread by poor hand hygiene. Proper hand hygiene is required of all FHP staff & providers, and we ask that patients, family members & other visitors practice hand hygiene while in our facilities. Hands should be cleaned with hand sanitizer before entering and after leaving a patient room, or washed with soap and water if visibly dirty.
Time out for procedures
As a universal protocol before all procedures, the team takes a "time out' to verify patient information, intended procedure and location of procedure.
Decreasing falls on hospital Medical and Surgical inpatient units
Patient falls are the most common adverse events reported in hospitals. We have implemented procedures to track the number of falls and best practices to lower the occurrence of falls.
Sepsis Management
Our sepsis team, composed of frontline staff and operational leaders, work to improve sepsis management at Fairbanks Memorial Hospital. Since October 2024, FMH has consistently met or exceeded the national average for sepsis bundle compliance. Sepsis-related mortality has decreased to "as expected" levels, representing a significant improvement in patient outcomes and safety.
