New FHP Patient Portal

Foundation Health Partners (FHP) is excited to announce the rollout of a new patient portal on June 1, 2026, that will give you access to all your FHP health and billing information on a single platform. If you have yet to do so, we encourage you to sign up for the existing FMH Patient Portal at: https://www.foundationhealth.org/fmh_portal. Enrolling now will allow you to be automatically enrolled in the new portal when it comes online in June.  

The new patient portal is a part of a larger project to give you and your provider improved access to your health information across the entire FHP health system through a new systemwide billing platform and Electronic Health Record (EHR) for our clinics. On June 1, all TVC, FHP Hospital Based Clinics, and Porter Heart & Vascular Center EHRs will move to the new system, which works more cohesively with Fairbanks Memorial Hospital and Denali Center.

Frequently Asked Questions

What is happening with the TVC/FHP Clinics and Porter Heart & Vascular Center’s Electronic Health Record system?

To provide you with improved access to your health information across the entire Foundation Health Partners (FHP) system, our clinics are moving to a new Electronic Health Record (EHR) and billing platform that works more cohesively with Fairbanks Memorial Hospital and Denali Center.

When will this take place?

The current launch date of the new EHR is June 1, 2026. We will send reminders and updates as we get closer to that date. 


Why is FHP making this change?

We are updating our system with the patient at the center of all decisions so that:

  • Your doctors and care teams can see your full medical history in one place
  • Information moves more easily between the hospital and clinics
  • Your care is more coordinated and efficient
  • We can better serve you in the future

This means one patient, one record across the entire FHP health system.

Will I need a new patient portal account?

If you currently have an FMH Patient Portal account, you will NOT need to create a new account. If you currently only use the TVC or Porter Heart patient portals, we encourage you to sign up for the FMH Patient Portal as soon as possible to be auto-enrolled in the new portal.

New portal features following June 1 will include:

  • Message your care team
  • Review visit summaries
  • Video visits with your provider (at participating clinics)
  • View test results

Can I enroll in the FHP Patient Portal before the new EHR launches?

Yes, all patients are strongly encouraged to enroll in the FMH Patient Portal prior to June 1, 2026. You can begin the enrollment process at: https://www.foundationhealth.org/patients_and_visitors/patient_portal/fmh

Will billing be affected?

Yes. FHP is also moving to a single billing system.   You will no longer receive separate bills from TVC, Porter Heart & Vascular Center, or FMH, as all billing will occur from a single system. You will still receive a separate bill for Emergency Department visits.

What will change in Billilng?

Your insurance coverage and financial responsibility are not changing because of this system update. Changes you may notice:

  • Updated billing statements
  • Slight differences in how statements look
  • Online bill pay through the new portal

Is my health information secure?

Yes. Protecting your privacy is extremely important to us.

The new EHR is a secure system that meets federal healthcare privacy and security standards.

Will my medical records still be available to my doctor/care provider

Yes. Your medical history from our previous system, NextGen Healthcare EHR, will be securely archived, and your providers will continue to have access to the information they need to care for you.

How can I request my medical records?

If you need to request your medical records, please go to: https://www.foundationhealth.org/patients_and_visitors/fhp_records. There you can request records from TVC, FHP Hospital Based Clinics, Porter Heart & Vascular Center, Fairbanks Memorial Hospital, and Imaging. 

What should I do to prepare?

There is nothing special you need to do. We recommend:

  • If you have not yet done so, enroll in the FMH Patient Portal
  • Arriving a few minutes early to appointments during the transition
  • Bringing your photo ID and insurance card
  • Watching for communication about the transition and the updated patient portal

Will this affect my upcoming appointment?

Appointments will continue as scheduled following June 1, 2026. However, if you have recurring appointments, please check with the registration team of your clinic to make sure your future appointments are reflected accurately in the new system.

Please note that following June 1:

  • Visits may take slightly longer during the first few weeks as staff adjust to the new system
  • You may be asked to verify your information
  • You may notice updated forms or paperwork

Will my doctor change?

No. Your providers and care team are not changing because of this system update.

What does this mean for FHP Hospital Based Clinics

If you receive care at one of our FHP Hospital Based Clinics (1st Care, Addiction Medicine, Behavioral Health, Endocrinology & Diabetes, General Surgery, Internal Medicine & Senior Care, OMM, Pain Treatment, Pediatrics, Urology) :

  • Your health records will now connect directly with hospital records
  • Test results and procedures will be more seamlessly shared among providers across the FHP health system
  • More accurate and transparent billing processes
  • You will receive one bill for all FHP services 
  • Your providers will have a more complete view of your health history

What does this mean for TVC Clinics (Family Medicine and Allergy & Asthma)?

If you receive care at TVC Family Medicine or Allergy & Asthma clinics:

  • Your health records will now connect directly with hospital records
  • Test results and procedures will be more seamlessly shared among providers across the FHP health system
  • More accurate and transparent billing processes
  • You will receive one bill for all FHP services 
  • Your providers will have a more complete view of your health history

What does this mean for the Porter Heart & Vascular Center ?

If you receive care at our Heart Center:

  • Your cardiology records will now connect directly with hospital records
  • Test results and procedures will be more seamlessly shared among providers across the FHP health system
  • Your providers will have a more complete view of your heart health history
