How do I enroll in the patient portal?

1. Provide a valid email address to your practice. You'll receive an invitation to enroll.

2. Create a username and password, then follow onscreen prompts to activate your account. 

3. Use your username and password to securely stay in touch with doctors and access your health information anytime, anywhere. 


How to request historical health records:

  1. Start by selecting "Health Records" on the Patient Portal Homepage.
  2. Once on the Health Records page, click the link labeled "Request Update Records."
  3. Choose either a specific date range or opt for the complete historical health record.
  4. Click on the "Request" button.

Please be aware that if your request is completed correctly, you will receive a confirmation message stating "Request Received." Once your records request has been processed, the information will be sent to your Patient Portal, and you will receive a notification.
