FLU VACCINE DEADLINE: Nov. 15, 2024


  • One significant change related to the influenza vaccine exemptions is they will be completed by appointment in the FHP OHS office. 
  • Per Joint Commission exemptions need to be renewed yearly. To book FHP Occupational Health (office365.com). 
  • The due date for vaccination or exemptions is the same as previous years Nov. 15th.   
  • The OHS clinic is located in the Doolittle Suite 219, hours of operation 7:00am-4:45pm.
  • OHS has established a few dates for night and weekend rounding offering the vaccine.
  • OHS will offer a flu vaccine clinic every Friday in the Miss Ghezzi Conference Room starting in October 7:30 AM - 2 PM. 

 

Flu Vaccination Requirements

Foundation Health Partners (FHP) requires that all employees, volunteers, students, licensed practitioners, non-employed contract personnel and vendors to receive the flu vaccine on an annual basis. Influenza vaccine is available free of charge, through Occupational Health Services, to all FHP employees, medical staff and volunteers. Denali Retail Pharmacy has the flu vaccine available for clinical contracted employees and students.     

Any individuals who become FHP employees or volunteers during the annual vaccination program must obtain the appropriate influenza vaccination, or exemption, for the current influenza season within two weeks of hire/start date.

On-site contractors and allied heath members not employed by FHP must provide proof of flu vaccinations, or exemption, during the designated flu season.


COVID 19

To best serve our community, FHP needs to ensure a healthy workforce. FHP requires employees to be vaccinated against COVID-19 (or receive appropriate exemption). 

There are many places to get the vaccine or booster, including Denali Pharmacy at FMH. (Click here to make an appt. with Denali Pharmacy.)
