Our Care

Our department recently launched a new care model to provide team-based patient care.

This new model aims to provide quality and timely care for immediate and ongoing patient needs. Your care will be provided by a physician-led care team. Routine check-ins, follow-up visits, and acute care services will be provided by an advanced practice provider, which might be a physician’s assistant or nurse practitioner. Your physician will offer consultations and assist with treating more complex issues.

Collaborating as a team allows us to expand our ability to care for more patients, reduce the wait time for appointments and be more responsive to patient needs. It gives everyone, at all ages, greater access to the care they need when they need it.

Frequently asked questions:

When will the clinic move?

Since Monday, January 9, 2023, we are located at 1919 Lathrop St. Suite 100 in the Doolittle Building, adjacent to Fairbanks Memorial Hospital.

Do I need to establish care at the clinic?

Whether or not you are a current patient within the Foundation Health Partners system, you will still need to establish care with the nurse practitioner's patient panel.

Will my provider change?

Under this new model, your care will be provided by a physician-led care team. Routine check-ins, follow-up visits, and acute care services will be provided by an advanced practice provider, which might be a physician’s assistant or nurse practitioner. Your MD or DO will offer consultations and assist with treating more complex issues.

Will it take longer to get an appointment?

We expect that this transition will provide greater access to care. Through collaboration and dedicated time to communicate patient plans, the new care model allows us to provide quality care in a timely manner, reduce wait time for appointments, and be more responsive to patient needs. Additionally, we’ve hired new providers to meet the demand of new patients accessing senior care.

I’m not a Senior, why am I being seen at a Senior Care Clinic?

Our providers specialize in a wide array of services that encompass both senior care and internal medicine. Although this clinic will see different populations of patients, there is a commonality between the two service lines, and that’s why they’ve been integrated into one clinic.
