About Us

Foundation Health Partner’s Senior Care Clinic offers specialized, primary medical care for senior patients. As we age, our medical needs change, so our healthcare should too. We’ve adopted a clinic model that is unique to the needs of seniors, allowing us to focus the appropriate level of attention on our patient’s individual health needs to deliver a truly personalized care experience. This model also allows us to preserve Medicare access in our community and increase the availability of appointments for patients who may need to be seen more frequently.  

Designed with seniors in mind, our patients no longer have to drive to multiple locations to get the care they need. We put their provider, specialists, testing, and imaging services on the same campus. Conveniently located on the first floor for a smooth car-to-care transition.

What We Do:

We provide comprehensive primary care for seniors, including annual physicals, regular checkups, and care for both acute and chronic conditions. Our team has extensive experience managing complicated medical conditions, including Alzheimer’s and dementia, depression, diabetes, heart disease, and other cardiovascular disorders, hypertension, stroke, and osteoporosis. 

Services we provide our patients include:

  • Advanced and long-term care planning
  • Behavioral health support, including senior counseling referrals services and depression screenings
  • Immunizations and vaccinations
  • Laboratory services, including diabetes screening and blood glucose testing
  • Osteoporosis screening, including bone mineral density (BMD) testing
  • Routine office procedures, electrocardiograms (EKGs), Kenalog joint injections, and simple skin procedures such as skin biopsy and cryotherapy.

Our Care

Our department recently launched a new care model to provide team-based patient care.

This new model aims to provide quality and timely care for immediate and ongoing patient needs. Your care will be provided by a physician-led care team. Routine check-ins, follow-up visits, and acute care services will be provided by an advanced practice provider, which might be a physician’s assistant or nurse practitioner. Your physician will offer consultations and assist with treating more complex issues. 

Collaborating as a team allows us to expand our ability to care for more patients, reduce the wait time for appointments and be more responsive to patient needs. It gives everyone greater access to the care they need when they need it.

How do I schedule my first appointment?

You can schedule an appointment one of two ways. First, by calling our clinic and requesting an appointment with a member of our reception staff (907-459-3570). Second, by filling out an appointment request form online.

How do I transfer my primary care?

Thanks to FHP’s integrated medical records system, transferring primary care from TVC Family Medicine is seamless. To initiate the transfer, call our clinic at (907)-459-3570 and request the change – we’ll take care of the rest. You do not need to call your former primary care provider to update them.
