Q: Why is the application not working or saving any information?

A: The application requires Adobe Reader and Internet Explorer or Safari to function properly.

Q: How do I apply for initial member privileges?

A: You can use the Initial Applicants or contact Medical Staff Services office with the request for the pre-screen privilege form at 458-5304 credentialing@foundationhealth.org

Q: What if I’m on staff and want a new privilege?

A: Send your written request to the Medical Staff Services office at credentialing@foundationhealth.org request and be prepared to provide any of the CME/Trainings/Certificate to support the request of privilege change.

Q: How much is my credential fee and when do I pay the fee?

A: Medical Staff Services requires payment to process applications. If you are employed by the hospital, contact the Medical Staff Services office regarding completion of payment.

Initial Application: Physician Fee - $400.00; Allied Health Professional - $200.00

Residents/FEMS provider – No fee

Reappointment: Physician Fee - $200; Allied Health Professional - $100

Q: How long will the credentialing process take?

A: The credentialing process may vary but typically an initial applicant takes approximately three months. The timeline depends heavily on the responsiveness of references, schools, affiliations, and the applicant’s timeliness submitting paperwork.

Q: Who actually reviews my file and decides if I have privileges?

A: The approval process includes a review and recommendation from the Department Chair, Credentials Committee, and Medical Executive Committee. Once those three bodies make recommendations, it is taken before the Board for the granting of privileges. This process at a minimum takes approximately five weeks from start to finish.

Q: Who can submit information?

A: The applicant or a designated point of contact (credentialing contact listed by the applicant on the application form). Please note, information must be primary source verified. This means that even though as a provider you submitted your transcript or certification, the Medical Staff Services team has to get the information directly from the source (i.e. school). 

Q: What do I need in order to become privileged?

A: All medical professionals, with the exception of military personnel, are required to hold an Alaska state license and carry liability insurance. The requirements for medical staff membership and privileges are outlined in the Medical Staff Bylaws and on the delineation of privilege forms. Allied Health Professionals will need a medical staff member to sponsor his/him for clinical privileges.
