Welcome to our Clinical Exchange page. There are two ways to submit a request for a clinical rotation at Foundation Health Partners:

1. The university submits the request.
2. The student submits an independent request.

Student Onboarding Requirement
All students attending a clinical rotation at Foundation Health Partners must complete the student onboarding process. A student is defined as anyone participating in a clinical rotation to receive educational credit.

University-Submitted Requests
If you are enrolled in a university program where rotations are managed by the institution, you do not need to request a clinical rotation yourself. Once the university receives approval for the rotation dates, it will guide you through account creation and onboarding. This process applies to:

• All in-state universities
• All undergraduate programs
• All graduate programs with a designated school coordinator

Student-Submitted Requests
Graduate students not enrolled in a partnered university who are seeking a unique rotation at Foundation Health Partners must submit an independent request. Submitting a request does not guarantee approval. All requests undergo a review process. This category typically includes:

• Provider-type students
• Osteopathic Manipulative Medicine (OMM) students
• Students working with community providers who will accompany them in seeing patients at Foundation Health Partners
